This must be filled out by a parent no later than January 10.

This must be printed, signed, and returned to WayPoint Church no later than February 28. It can also be returned via email to [email protected]. A digital signature on a PDF using a tablet is also acceptable.
*If you had previously signed a waiver before camp was postponed, we only need the first page re-done.
